PHP Form Wizard demo

PHP Form Wizard is a tool for create or load an html form from other pages. When we open the program we can see two box, one of them is for load an html form and the other box is for Load a Html Form From URL. The other options that we can select are: Send email to owner´s form with all fields data, Send a confirmation email (autoresponder), Insert fields data into a MYSQL table, etc.
